Who is home care for?

Home care suits anyone who wants to remain in their own home but needs some level of support. That might be:

  • An older person who’s still independent but could use help with cleaning, shopping or transport
  • Someone returning from hospital who needs short-term or restorative support
  • A person with chronic conditions needing regular nursing or allied health
  • A family caring for a loved one who needs respite or shared support
  • A person living with early-stage dementia who can stay home with the right scaffolding

How Home Care Works at My Evervale

1. Initial conversation: Call us or submit a referral. We’ll talk through your situation and what you’re hoping for.

2. My Aged Care assessment: If you haven’t already, we help you get assessed via My Aged Care to determine eligibility and funding.

3. In-home consultation: A care coordinator visits to understand the home, the person and the family. We co-design the care plan with you.

4. Care begins: Your matched carer or nurse starts. Most clients have services in place within 1–2 weeks of agreement.

5. Ongoing review: We review your care plan regularly. As needs change, we adjust services — within My Evervale, no provider switching needed.

Home Care Funding & Costs

Most home care is funded through one of:

  • Commonwealth Home Support Programme (CHSP) entry-level support with a small client fee
  • Support at Home Program (from 1 July 2025) replaces Home Care Packages, with funding tiers based on assessed need
  • DVA for veterans and war widows/widowers
  • Private pay for those who don’t qualify or want services beyond their funded plan
