1. Initial conversation: Call us or submit a referral. We’ll talk through your situation and what you’re hoping for.
2. My Aged Care assessment: If you haven’t already, we help you get assessed via My Aged Care to determine eligibility and funding.
3. In-home consultation: A care coordinator visits to understand the home, the person and the family. We co-design the care plan with you.
4. Care begins: Your matched carer or nurse starts. Most clients have services in place within 1–2 weeks of agreement.
5. Ongoing review: We review your care plan regularly. As needs change, we adjust services — within My Evervale, no provider switching needed.
