What 24-hour care actually means
“24-hour” care can mean three different things. The right model depends on your situation:
- Live-in care — one or two carers rotating in shifts who live in the home. Best for full-time companionship and personal care.
- Sleepover (passive overnight) care — a carer present overnight, available if needed but allowed to sleep. Best when overnight needs are infrequent.
- Active overnight care — a carer awake and providing continuous care. Best for end-of-life, post-surgery and complex dementia.
When families choose 24-hour home care over a residential home
- The person has lived in their home for decades and a move would cause distress
- A spouse with capacity is also at home and shouldn’t be separated
- The person has complex cultural, dietary or pet needs that residential care can’t meet
- The family wants to stay involved in daily care
- End-of-life many families choose to keep their loved one home for the final months
What 24-hour care includes
- All personal care (showering, dressing, toileting, mobility)
- Continence management overnight
- Medication management to schedule
- Meals prepared to dietary needs
- Light domestic work (laundry, beds, kitchen)
- Companionship and cognitive stimulation
- Transport to appointments
- 24/7 escalation pathway to a registered nurse and the on-call clinical lead
Clinical supports we layer on top
- Registered nurse visits as scheduled (and on demand)
- Wound care, catheters, drains, syringe drivers
- Palliative care in coordination with your GP and palliative service
- Allied health (physio, OT, podiatry, dietetics)
- Telehealth links to your GP for after-hours questions
Funding 24-hour care
True 24-hour care is rarely fully funded by a Home Care Package or Support at Home alone — it’s typically a hybrid:
- Government-funded clinical and personal care delivered through your package
- Privately-funded companionship and overnight presence (typical out-of-pocket for live-in: ~$1,200–$2,500 per week depending on need)
- End-of-life situations may attract additional state-government palliative funding
